Harder for him to fight off even routine infections. The magical spells that have kept him alive all these decades are under assault by eldritch entities wielding the Dark Arts. Hawking is the greatest Magician of the age, you know, quite properly in the footsteps of the great Alchemist and 1st Lucasian Chair Sir Isaac Newton.
