Jason Nolte, 31, suffered fatal traumatic injuries on Saturday, June 21 while working at a window company in Aurora, Colorado. Local stations KMGH, KDVR and KUSA provides some initial information on Nolte’s death:
The incident occurred at about 8:30 a.m. at Manko Window Systems
Nolte was helping to unload two crates of glass
Nolte was crushed by the 4,000 pound load
A former employee of the facility said it wasn't the first time that glass fell on a worker at the plant. He said he'd give their safety program a D-minus grade.

Roxanne Moyer wondered why managers at her husband's worksite would allow an obvious dangerous condition to exist. Workers could be so "close to molten steel [that it] just poured over on them." Her husband, Samuel Moyer, 32 died earlier this month at Arcelor Mittal's LaPlace, Lousiania steel mill in exactly that way. He was fatally burned with molten steel.

This year we haven't had a lot of news about volcanic activity in Indonesia. This is not to say that eruptions haven't been happening, rather they just haven't been in the news. If you check out the current status of the volcanoes of Indonesia, you'll see that no less than six volcanoes are on orange (Level 2) status and another fourteen are on yellow (Level 1 - and there is a Level 0 as well). Ash fall on a taxi cab near Guatemala's Pacaya.
Pacaya in Guatemala erupted yesterday causing evacuations of people near the volcano and the closure of Guatemala City's main airport.
