Under the circumstances, I think it's a reasonable reaction.

Of COURSE that guy looks sad! Imagine having to hold that painful pose for the artist -- especially with a knife in the head!

The top guy looks sad. The bottom guy looks horrified.

Well Duh, CPP. Wouldn't you look horrified if not only were people operating on you while you're awake, but the people poking around in you were small children?!?!?
